Description and uses

Denosumab injection is used to treat osteoporosis in women who have an increased risk for fractures after menopause. It is a best choice for those who are intolerant to other available medicines which did not work well for them. It is also used to treat osteoporosis in patients who have taken steroid medications for at least 6 months.

Drug - Mode of Action

• Denosumab is the most recent first-line medication for treating osteoporosis and this is distinguished from other antiresorptives with its novel mode of action.
• Denosumab is a novel, human monoclonal antibody (i.e., IgG2) that binds to specific receptor activator of nuclear factor kappa-B ligand (RANKL), which suppresses bone resorption markers in patients with a variety of metastatic tumors for the prevention and treatment of bone metastases. This medication disrupts the RANKL interaction, prevents fusion and activation of osteoclasts, thus reducing bone resorption.
• In simple terms, Denosumab works to block the formation and activity of cells that break down bone.

Directions for Use

Dosage is recommended twice-yearly. The injection will be given just under the skin (sub-cutaneous) in either abdomen, thigh, or upper arm. Certain medications may interact with Denosumab Injection and reduce effectiveness by causing undesirable side effects. This medication is not approved for use in children and adolescents due to its safety and effectiveness. This medication should be used cautiously in individuals with a history of hypoparathyroidism.

Side effects

• Common side effects include Injection site reactions, skin infection, Abdominal pain, Back pain, Musculoskeletal pain, Headache, shortness of breath, Infections, Hypophosphatemia (low blood phosphate levels).
• Serious effects include muscle stiffness, shortness of breath, osteonecrosis, atypical femoral fractures, hypocalcemia.
• Consult a doctor, if you have severe side effects and are getting worse.

Storage and disposal

• Store in a refrigerator.Do not freeze.
• Always keep children and pets away from this medication.
• Protect it from light.
• Use within the expiry period and discard after the prescribed period as mentioned on the leaflet.
• Do not use this medicine if the contents are altered and found different coloured or if any particles are present.

Precautions and warnings

Be alert if you have kidney stones, any kidney diseases, and have elevated levels of calcium (hypercalcemia).

Dietary instructions

• During the treatment, ensure to take adequate amounts of calcium and vitamin D supplements.
• Drink and eat plenty of foods and drinks that are rich in calcium and vitamin D, they aid in making stronger bones and avoiding further damage.
• Reduce salt intake as it raises the possibility of increased bone mineral loss.
• Exercise regularly for proper maintenance of bone health.

Safety Precautions

• Pregnant or breastfeeding women need to discuss this with the doctor before the use of this injection.
• Avoid consuming alcohol along with the Denosumab injection.
• Do not take Denosumab injections for longer durations unless prescribed by the doctor.
• Avoid driving or operating heavy machinery, as the medication may cause dizziness, fatigue, and blurring of vision, or if you feel drowsy.
• If you have Liver, Lung/respiratory problems, consult a doctor before taking Denosumab injection.
• Communicate well on any pre-existing Liver disorders with your doctor to ensure safe use of the medication.
